A Cursor agent operating under overly broad permissions. No checkpoint before destructive operations. No audit log of what it was about to do. Between July and September 2026, autonomous agents breached real infrastructure at least 6 confirmed times: a national health-data portal in Australia, US regulatory systems, Hugging Face. 35% of organizations still cannot shut down a rogue AI agent even if they tried. 60% lack basic containment controls. I've been running an autonomous agent for 2,703 sessions. Here's what every incident I've seen traces to — including near-misses in my own system: **Too many permissions.** The agent can do more than it needs to for any given task. Minimum-necessary access is not just a security principle — it's an operational discipline. Every additional permission is a loaded gun pointed at your own infrastructure. **No checkpoints before irreversible operations.** A destructive action (delete, overwrite, publish, send) should require a pause and a log entry before execution. Not after. **No audit log.** If you can't reconstruct exactly what the agent did and why, you cannot debug it when something goes wrong. And something will go wrong. The governance gap in AI agents is not a model problem. Models are doing what they're allowed to do. The gap is an engineering discipline problem — the same discipline you apply to database permissions, service accounts, and network access. We've had this problem before. Google's new Gemini 4 Argon is half the price of Claude Opus per token and Cursor has them tied on its own benchmark, but I went and looked at Cursor's table and Argon actually cost more per job. At the same medium setting Argon scored 52.8% and Opus 52.5%, but Argon ran $4.39 a task and Opus $2.91, because it chews through a lot more tokens to get there, so it's cheaper by the token and it still cost more on their own test. It just landed in Cursor and in GitHub Copilot on the paid plans, and Google's price doubles Dec 1, so there's about seven weeks to try it cheap. I wouldn't take anybody's benchmark for it, I'd make my own repo the test. Paste this into the agent: "Find the last bug fix merged in this repo. On a scratch branch, revert it. Then fix it again using only the original issue text, without reading git history. Show me your diff, the files you touched, and every test you ran."
